BACKGROUND

The COVID-19 ASSESSMENT CENTRE was co-designed and co-led by the East Toronto Family Practice Network and Michael Garron Hospital.

The program’s goal is to provide assessment and screening in an ambulatory care setting for patients who present themselves to the clinic located in the D-Wing, first floor at Michael Garron Hospital.

The main objectives of this initiative include:

  • provide East Toronto residents with access to COVID-19 assessment and screening

  • support low acuity screening adjacent to the Emergency Department (ED) to help alleviate volume pressures.

  • facilitate and better coordinate follow-up care – either to primary care or the ED.

CLINIC INFORMATION

  • Hours of Operation - 7 days per week from 8:00am to 8:00pm ( Opened to Public on March 11, 2020 ; Expanded Hours beginning March 16)

  • Located at Michael Garron Hospital at 825 Coxwell Avenue, D-Wing with single entrance/exit at Mortimer Avenue (8am-8pm)

    • Note: The MD/NP registering for the noon to 4pm shift may occasionally be required to drive to a partnering Long Term Care facility, Retirement Home or shelter, where you will be fully supported with PPE/NP swabs and an outreach team where applicable as part of the Centre. If you book the noon to 4pm shift, you will be required to provide your own transportation to support the outreach.

  • Shifts available for clinical staff in 4 hour block minimums - with flexibility to select your preferred dates

SUPPORTS WITHIN THE CLINIC

  • Receptionist for screening and onsite security

  • Three Emergency Triage Nurses (Vital Signs, Document and Patient Education)

  • Clinical Rooms with Personal Protection Equipment Provided

  • Access to Specialist Advice and Supports
