This secure messaging platform allows PCPs to connect with specialists regarding mutual patients, consult specialists on-call for urgent concerns, and connect with the SCOPE nurse navigator to access specialists and mental health resources more easily.

MGH credentialed clinicians can access the tool for free through MGH.

Primary care providers who are new to practice in East Toronto (within the last 1-2 years) can access the tool free of charge for one-year through EasT-FPN (if not already credentialed at MGH). Please email us at contact@eastfpn.org to indicate your interest or if you would like more information.

OCEAN eReferral

eReferral simplifies the referral process to community- and hospital-based specialists and services. It integrates directly with several EMRs, allowing you to send referrals quickly & receive updates within your integrated EMR or a web-based portal. More than 600 Toronto-based specialists and clinics are receiving e-referrals, and more than 140 family doctors and nurse practitioners in East Toronto are actively using the platform.

SCOPE

SCOPE provides direct access to navigation and a range of specialists in a timely manner to help primary care providers provide the best care to their patients, so as to best support the day-to-day work of primary care.

EasT-FPN continues to support the expansion of the SCOPE program for family doctors and nurse practitioners in East Toronto.
