Roadmap

 

East Toronto Family Practice Network was built through a deliberate, physician-led process of engagement, governance development, and strategic partnership.

  • Our roadmap reflects the key milestones that have shaped EasT-FPN’s growth — from initial physician mobilization to becoming an established and influential voice for family practice within East Toronto’s evolving health system.

  • As our region and our members’ needs continue to evolve, so too does our work.



Stage 1: Building a Collective Voice

Goals

To engage family physicians and primary care clinicians across East Toronto in support of creating a unified network for family practice leadership and advocacy.

Key Outcome

By November 2019, more than 190 of East Toronto’s 270 family physicians had signed letters of support endorsing the creation of EasT-FPN and its vision for stronger physician collaboration.

This early engagement established a clear mandate: family practice wanted — and needed — a stronger collective voice.


Stage 2: Establishing Strategic Partnerships

Goals

To define EasT-FPN’s role within the broader East Toronto health system and build formal relationships with existing regional partners.

Key outcome

EasT-FPN entered into foundational agreements that formalized its participation in regional planning and clarified the conditions under which family practice would engage in Ontario Health Team development.

This ensured that primary care representation was embedded from the outset.


Stage 3: Advancing Ontario Health Team Integration

Goals

To work collaboratively with East Toronto Health Partners in the successful development of an Ontario Health Team application.

Key Outcome

EasT-FPN became a signatory partner in the East Toronto Ontario Health Team application, helping secure family practice representation within one of Ontario’s early integrated care models.

This milestone positioned EasT-FPN as a recognized contributor to regional health system transformation.

Stage 4: Strengthening Internal Governance and Physician Community

Goals

To establish EasT-FPN as a sustainable, physician-led organization with strong internal governance and broad member engagement.

Key Milestones

  • development of formal bylaws and membership criteria;

  • election of a Board of Directors;

  • establishment of an Executive Committee;

  • physician town halls and engagement forums;

  • development of operational and strategic infrastructure.

These foundational steps have enabled EasT-FPN to move from concept to long-term organizational leadership.

Stage 5: Collaborating for Community Impact

Goals

To work alongside physicians, hospitals, community organizations, Ontario Health, and East Toronto Health Partners on initiatives that improve patient care and support family practice sustainability.

Our Ongoing Focus

EasT-FPN continues to pursue collaborative projects that align with the Quadruple Aim:

  • improving patient experience;

  • improving population health;

  • enhancing provider experience;

  • supporting system sustainability.

As physician needs and health system priorities shift, EasT-FPN remains responsive, adaptive, and committed to practical action.

 

Looking Ahead

Our roadmap is not static.

EasT-FPN continues to evolve through member engagement, strategic advocacy, and partnerships that strengthen comprehensive primary care across East Toronto.
