About EasT-FPN
The East Toronto Family Practice Network (EasT-FPN) represents the voice of over 240 primary care providers in East Toronto. As part of the East Toronto Health Partners, the EasT-FPN acts as a primary care anchor partner for the East Toronto Ontario Health Team. The EasT-FPN fosters connection and partnerships with local family practices and care teams, to support and improve integrated care delivery pathways, the communication and knowledge sharing of primary care information to meet the needs of the East Toronto community.
Position Overview
The Digital Health Consultant will support digital transformation across community family physicians and primary care practices in East Toronto. The primary function of the role is to provide direct support to community primary care providers with the onboarding, training, optimization, and change management of digital health tools, to advance the overall level of digital tool utilization across the membership of EasT-FPN.
Core Responsibilities
1. Digital Health Support for Community Primary Care Providers:
Develop and maintain a list of EasT-FPN supported digital tools (HRM, Clinical Viewer, eReferral, OLIS, EMR integrations, Ocean, E2P pathways) to assist practitioners. Troubleshoot technical issues from digital tools as reported by EasT-FPN practitioners and escalate resolution to vendors or regional supports on behalf of EasT-FPN members.
Develop and maintain a list of external qualified digital consultants to assist practitioners.
Create and manage an intake system for digital support requests from practitioners.
Respond to practitioner requests for digital health support for EasT-FPN supported digital tools. Provide practitioners with preliminary assessment of work effort required and timing to align and tailor EasT-FPN supported digital tools to each practitioner’s practice including use of external digital resources and consultants where appropriate.
Directly implement, train, troubleshoot and provide change management for digital tools to practitioners on an equitable basis to extent possible based on workload.
Support digital workflow optimization tailored to different practice sizes.
Support EMR optimization, roster management, and use of population health reports.
Facilitate in-clinic and virtual support for adoption and optimization.
2. Support for EasT-FPN & ETHP Digital Initiatives:
Collaborate with ETHP digital and primary care leads.
Support evaluation, tracking, and reporting on digital adoption and impact.
Monitor provincial digital health developments and identify opportunities for primary care.
3. Relationship Management & Collaboration:
Work closely with the EasT-FPN Project Coordinator to align digital support with engagement activities.
Engage clinicians and staff to build awareness and uptake of digital tools.
Attend EasT-FPN and OHT meetings as required.
Qualifications
Experience:
2–5 years in healthcare office management, preferably primary care office management working with family doctors and other primary care providers.
Experience providing troubleshooting support in primary care office settings.
Experience supporting digital health implementations or EMR optimization.
Experience with PHIPA and privacy best practices.
Familiarity with Ontario digital tools such as HRM, OLIS, Clinical Viewer, Ocean, eReferrals, and major EMRs.
Experience with workflow redesign or change management.
Skills:
Strong communication and relationship-building skills.
Ability to manage multiple cases independently.
Excellent problem-solving and analytical ability.
Strong organizational and time-management skills.
Comfort with technology and rapid learning of new tools.
Valid Ontario driver’s licence and access to transportation.
Education:
Degree in Health Informatics, Health Sciences, Computer Science, Information Management, Business/Technology, or a related field is an asset.
Compensation
$97,000 - $106,000 per year - Full time
Work Environment
Hybrid work model with frequent travel to community primary care offices throughout East Toronto.
Standard 37.5-hour work week with some flexibility.
