Our vision and work means it’s important to have a website that clearly and comprehensively communicates what the EasT-FPN is doing. Our website is our best way to communicate to our stakeholders, which includes family physicians, our community partners, and potentially even patients.

If you’re doing work with the EasT-FPN, or if you’re part of our community more generally, you are welcome to submit requests for updates and changes to our website using the following form.

For example:

  • If you’re involved in the Digital Health Workstream and that page needs to be updated, submit the form!

  • If you’re a physician in East Toronto, and you think our COVID-19 page is missing information, submit the form!

  • If you’re a health care provider in East Toronto, and you think some key information should be added, submit the form!

All submissions will be reviewed, and we’ll reach out as required if follow-up is needed.
