Help Connect East Toronto Residents to Primary Care

Join the East Toronto Primary Care Attachment Program

The East Toronto Health Partners (ETHP) Ontario Health Team (OHT) is working with primary care and community partners across East Toronto to improve access and attachment to primary care for residents who do not currently have a family doctor or nurse practitioner.

Through this coordinated initiative, providers can help connect unattached residents to primary care based on capacity, location, health needs, and language or cultural considerations.

About the Program

Healthcare organizations across East Toronto are working together to support equitable access to primary care. A central intake team matches patients to participating providers and clinics in the community.

Who Can Be Referred?

Patients who:

  • Live in East Toronto

  • Do not have a family doctor or nurse practitioner

  • Are experiencing barriers accessing existing primary care provider 

Referral and Attachment Process

Providers can refer patients to ETHP Central Intake through established referral pathways.
