EasT-FPN 5TH ANNUAL GENERAL MEETING

OCEAN e-REFERRAL UPDATES

  • University Health Network (UHN) is accepting Ocean eReferrals for multiple departments:

    • Blood Disorders Program - Comprising three clinics: General Hematology, Red Blood Cell Disorders, and Thrombosis & Hemostasis Clinic, this department is dedicated to managing diseases of the blood and bone marrow.

    • Francis Family Liver Clinic - Offers services to patients with all types of liver diseases, especially hepatitis B, hepatitis C, fatty liver, autoimmune liver disease, and patients with advanced liver disease requiring complex care and long-term follow-up.

    • Dermatology - Provides treatment to many different skin and nail conditions such as psoriasis, atopic dermatitis, skin cancer, pemphigus vulgaris and bullous pemphigoid, cutaneous lupus, and medical and surgical nail disease.

  • MGH continues to accept referrals via eReferral across all specialties and departments, including referrals for their Internal Medicine Medical Assessment Clinic, and for their mental health specialist at Child and Youth Mental Health Services and Outpatient Mental Health Services (Adults)

SCOPE MENTAL HEALTH PROGRAM UPDATES

  • Time-limited counselling/therapy (up to 8 sessions, with a Social Worker or Registered Psychotherapist), for clients experiencing anxiety, depression, emotion regulation issues, grief, relational issues, OCD, trauma, etc.

  • Refer patients through the preferred method of communication, Ocean E-referral as it provides easy, direct and confidential contact with the mental health counsellors/navigators.

  • Wait times will apply and your patients will be notified of estimated wait times when the referral has been received. You will be advised once your patient has been added to the waitlist and again at the time of discharge from short-term counselling. Should you require additional updates, please connect with the team directly via email or phone.

  • Integrated Care Pathways is currently at the Design and Build Stage and the COPD pathway is expected to go live in early Fall.

1. Integrated Care Neighborhood Project - Primary Care Leadership Position

  • The East Toronto Family Practice Network (EasT-FPN), together with East Toronto Health Partners (ETHP), is in the process of recruiting a Primary Care Lead, to support the delivery of homecare, and the integration of other services in the Taylor Massey and Thorncliffe Park neighborhoods.

  • The Primary Care Lead (s) will work with homecare teams, to co-design care pathways that connect patients to home care, community care services, and other ETHP care models.
