Register for the AGM Today!

  • Have you registered yet for our 3rd Annual General Meeting on Sept 28 from 7-9pm? Register today to attend either in-person or virtually to network with primary care colleagues and elect new EasT-FPN Board members.

Women’s Withdrawal Management Services

  • Michael Garron Hospital’s new Women’s Withdrawal Management Services (WWMS) is now open to clients at 989 Danforth Ave. The facility supports women-identifying individuals with mental health and substance use concerns. 

  • Learn more about MGH’s WWMS, including how to refer clients and community members.

Memo on Aconitine Poisoning

  • Please click here to view the memo created by the Ministry of Health which outlines symptoms and what to do if an individual presents with Aconitine poisoning.

Community and Specialist Resources

  • Multidisciplinary Clinic in East Toronto with SCOPE spots available - Leaside Village Medical Clinic. Services include, but are not limited to: cardiology and cardiac diagnostics, urology with a special focus on sexual medicine and andrology, dermatology with a specific focus on worrisome lesions/moles and the ability to do non-facial in house surgical resections, and physician-supervised IV-Iron treatment.

  • Ocean eReferral continues to expand in East Toronto – if you are interested in using Ocean eReferral, please contact the eServices Team: eReferral@ehealthce.ca.

  • Become an EasT-FPN member to receive clinical and practice support, and opportunities to engage with the East Toronto community. Please fill out the Membership application form to become a member.

    • As part of the membership onboarding, members are encouraged to complete the following EasT-FPN Membership Intake Form, to provide us with key practice information, which will inform EasT-FPN program and service planning
