Coached Corridor Initiative

  • We want our community to flourish. We want you to thrive. The Coached Corridor initiative aims to reshape the way we connect patients to family doctors by focusing on physician well-being first. 

  • We’re giving you the opportunity to be paired with a professional coach at no cost who will help you reach your personal and professional goals!

  • The fully funded initiative is available to 12 family physicians in East Toronto. Acceptance to the program takes place on a rolling, first-come, first-served basis. The deadline to apply is Monday, January 24th at 5 p.m.

Save the date: Virtual Mix and Mingle Family Practice and Specialists - March 2, 2022 from 6-8pm

  • Take a break from COVID-19 and come meet family practice and specialists from all over East Toronto!

  •  Join your colleagues for a virtual networking opportunity and learn more about the benefits of SCOPE, Oceans eReferral, and Hypercare that all aim to make referrals more seamless in East Toronto! Stay tuned for additional details. 

Caregiver NICE Fund: Applications now open!

  • This initiative is for East Toronto’s low-income caregivers, with the goal of helping them and their care recipients through the “Surge” months by providing basic necessities and services. More information and how to apply can be found using this link.

Schedule N-95 Mask-Fit-Testing for Family Practice

  • Schedule your front-line staff and physicians for N-95 Mask-Fit-Testing using this link for individuals and this link for practices (25-35 staff). 

COVID-19 Response Forums

  • Primary Care COVID-19 Open Forum
    Join us for the monthly primary care open forum on January 26th from 6:00-7:00pm for COVID-19 updates, resources and a Q&A opportunity with Dr. Jeff Powis, Medical Director, Infection Prevention and Control at MGH. Click here for the Q&A from the last forum.

SCOPE 

  • Register for SCOPE today and get rapid access to nurse navigation, general internal medicine, diagnostic imaging and more! 

  • Please email SCOPE@tehn.ca with any feedback/suggestions for new services you would like to see added.

  • Download Hypercare for easy and secure communication among community primary care providers and hospital care providers. Visit the EasT-FPN website for more information on digital solutions and Hypercare.

Become an EasT-FPN member

  • The EasT-FPN encourages you, if you have not already, to become a member to receive clinical and practice support and opportunities to engage with the East Toronto community. Please fill out the Membership application form to become a member.

Membership Intake Form

  • As part of the membership onboarding, members are encouraged to complete the following EasT-FPN Membership Intake Form, which will provide us with the necessary information to coordinate access to interprofessional digital services and platforms and provide us with key practice information, which will help inform EasT-FPN program and service planning for East Toronto primary care practices.
