Outpatient Treatment for COVID+ Patients - This Wednesday February 9 at 1pm
Scarborough Health Network is featuring a session from 1-2 pm on how to obtain care for your patients with COVID at SHN’s therapeutics clinic. Drs. Susan John, Elaine Yeung and Adbel Belhaj will describe the various aspects of diagnosis, referral, and clinic treatment as well as long-term care access for your patients with COVID. Glyn Boatswain, VP and Chief Nursing Executive will describe how SHN Assessment Clinics operate.

Complex Personality Presentations: Education Series hosted by Primary and Community Care Response Teams (PCCRT)
Part 1 – “Understanding personality formation, attachment and the language around personality disorders” (Friday February 11 9:00-10:00am)

Facilitated by Dr. Mark Lachmann, a geriatric psychiatrist working with patients in acute, rehabilitation, long-term care, and community settings. Mark is a member of the Primary and Community Care Response Teams' Seniors Mental Health Collaborative, where he shares his expertise to support integrated care planning for adults with unmet health and social needs. He practiced as a family physician for ten years in Northern Canada before re-training as a psychiatrist.
Primary and Community Care Response Teams (PCCRT)
Offers team-based health and social services care planning to help vulnerable adults navigate the healthcare system.
Adults who may not have a family doctor, may be housebound and/or have unmet health or social needs are connected to additional supports and services that are close to home.
An ETHP initiative that includes family physicians, home care coordinators, nurse practitioners, case managers, mental health practitioners, supportive housing workers, volunteer community health ambassadors and others.
By working together within their local communities, our PCC Response Teams create integrated, comprehensive coordinated care plans for vulnerable adults that help them meet their health goals at home and in their community

As part of the membership onboarding, members are encouraged to complete the following EasT-FPN Membership Intake Form, which will provide us with the necessary information to coordinate access to interprofessional digital services and platforms and provide us with key practice information, which will help inform EasT-FPN program and service planning for East Toronto primary care practices.
