We need your Help!
The EasT-FPN, together with the Department of Family and Community Medicine, is conducting a comprehensive census to inform primary care capacity planning in East Toronto, led by Ontario Health.
We need to gather the following information from primary care providers to sustain, expand, and enhance primary care capacity and access in our communities:
Who are we?
Where do we practice?
How do we practice? (team/no team)
What do we do? (home visits/walk in)
When do we practice?
What, when, where and how do we access our current resources?
What are the current and future needs?
The Census
The census will take approximately 20 to 30 minutes to complete and will be available until March 23, 2025.
EasT-FPN is seeking YOUR support
We need to speak with all family physicians and nurse practitioners in East Toronto to do a deep dive, learn about the populations they serve, the barriers in the system and improvements needed to keep primary care functioning.
Please help us link with your colleagues. We need to identify point person(s) in your neighbourhood to help us with:
Identify who should complete the census
A “warm link’ either via email/ phone
Census distribution (email, fax, social media)
If possible, arrange small in-person or virtual meet-ups (eg. lunch/coffee)
EasT-FPN will offer a stipend for those that complete the census and those that can help us champion this work.
