HOW TO ACCESS YOUR COVID-19 VACCINE:
The Ontario government has identified eligibility for a third dose of the COVID-19 vaccine. Based on the eligibility, Michael Garron Hospital (MGH) has provided information on how to book an appointment for 1st, 2nd or 3rd doses, and where the various vaccination clinics and pop-up vaccine clinics are located in East Toronto.
Toronto Public Health Mass Immunization Clinics: To get vaccinated at one of the city-operated immunization sites, please register using the province’s vaccination registration system.

HOMEBOUND PATIENT VACCINATION DRIVE
Does your homebound patient require the COVID-19 vaccine? MGH and ETHP are vaccinating homebound individuals in East Toronto to offer the COVID-19 vaccine. Patients must reside in the MGH catchment area: M1L, M3C, M4A, M4B, M4C, M4E, M4H, M4J, M4K and M4L.

PRIMARY CARE VACCINE CLINICS
Are you interested in running a COVID-19 vaccine clinic in your practice? Toronto Public Health (TPH) has moved to a new system for the Primary Care COVID-19 Vaccine Program, allowing for more sites to join the program, and a more streamlined ordering experience.
Complete TPH's updated Enrollment Request Form, as practices must be enrolled into the program prior to ordering

Sign up for a shift at the COVID-19 Assessment Centre and Vaccination Clinics shifts - please see the steps on how to sign up for a shift below:
Must be credentialed at MGH – click here for instructions
Review roles and responsibilities and counseling guidelines for shifts at the CAC and vaccination clinics
Vaccination clinic shifts consist of administering the vaccine AND/OR answering questions from patients about the vaccination(s) where you must be comfortable and willing to take on either of these roles and must be knowledgeable on different vaccines, side effects, C/I etc. The EasT-FPN has multiple online resources regarding COVID-19 vaccination available here
