East Toronto OHT Application

The EasT-FPN signed on to the East Toronto OHT application on October 9, 2019. The East Toronto OHT application from the East Toronto Health Partners was successful in moving to the next stage in the application process. A site visit from the MOHLTC Community Outreach Team was conducted on October 29, 2019. The East-FPN was invited to attend. Drs. Catherine Yu and Kevin Workentin presented on our emerging partnership and how we can work together to advance our unique model in East Toronto.
