Holistic Intake & Navigation Counsellor (HINC) - 2 positions

We are thrilled to invite you to join our NEW Interprofessional Primary Care Team, aimed at improving healthcare access and attachment for residents of Thorncliffe and Flemingdon Park neighbourhoods. Our services will be extended to various locations with expanded hours to better serve our community.

Our enhanced care team will offer a range of services to ensure comprehensive primary care, social prescriptions, and population health initiatives. We will provide attachment to comprehensive primary care for those in need, facilitated by a multidisciplinary team comprising MDs, Holistic Intake and Navigation Counsellors, Peer Support Workers, and RNs. These services will be accessible through various points in the community, making it convenient for residents to receive care. Health Access Thorncliffe Park (HATP) is an innovative partnership between Flemingdon Health Centre (FHC) and The Neighbourhood Organization (TNO), and we are seeking 2 Holistic Intake and Navigation Counsellors to join our team in full-time Contract roles. TNO is a not-for-profit community based multicultural and multi-service agency, located in Toronto, Ontario, with a vision of neighbourhoods where everyone belongs and succeeds.

The position involves providing culturally sensitive services to clients, including intake assessments managing waiting lists, and orienting new clients, Responsibilities include maintaining accurate documentation, reporting on client intake, referrals, and community connections. Acting as a single point of access, the role includes triaging calls and connecting clients with services. Collaborating with other providers, the role aims to streamline client journeys and stay informed about service offerings. Additionally, the worker will identify factors impacting health, create personalized support plans, connect clients with community groups, provide crisis counseling, and liaise with service providers. Outreach and participation in community development activities, such as advocacy, engagement, and supporting local events, are also essential.

The ideal candidate should have a Bachelor of Social Work (BSW) or equivalent education and 1-3 years of experience in frontline services for newcomers/equity deserving communities. They need strong analytical and decision-making skills, crisis intervention abilities, and excellent communication in English, with a second language as an asset. Organizational skills are crucial for managing varied workloads and responding to crises promptly. Interpersonal skills for liaising with professionals and staff, cultural sensitivity, active listening, problem solving, and proficiency in computer use are necessary. Additionally, the candidate must pass a criminal record check and be willing to conduct neighborhood home visits.

Main Duties & Responsibilities

  • Provide culturally sensitive services and information on the HATP model of care and community service.

  • Complete accurate documentation, provide reports, and perform administrative duties.

  • Intake: Conduct assessments, provide information, screen, enroll, and orient new clients.

  • Navigation: Refer to resources, offer support, and crisis counseling to clients.

  • Serve as a single point of access, maintain reports, streamline client journeys, and participate in outreach and community development activities.

Vaccination Requirement – You must be fully vaccinated against COVID-19, subject to any valid medical or religious exemption from vaccination as provided by the Ontario Human Rights Code.

Screening – As a condition of employment, the successful applicant must complete a Vulnerable Sector Screening.

Job Type: Full Time, Contract until March 31st, 2025, with possibility of extension

Schedule: It is expected that the incumbent will work a regular schedule for 35 hours per week (7 hours per day, excluding the lunch hour), with flexibility between 8.30 am and 8 pm on weekdays, and weekends as required by the HATP/TNO programs/departments
