Virtual Care Guidance for Primary Care

  • Ontario Health has recently released new virtual care guidance materials to support primary care clinicians in making decisions related to the planning and delivery of virtual care (via different modalities, including phone, video and email/messaging)

  • Includes customizable patient-facing materials to help patients understand the benefits and limitations of different modalities when seeking care (available in English and French, but will be translated into a number of other languages common to East Toronto soon!)

  • Please review these guidance documents, and contact us at: contact@eastfpn.org if you have questions or feedback for future iterations

SCOPE Mental Health Program

  • The SCOPE Mental Health Program provides rapid contact for patients in need of mental health support

  • Comprised of a team of two mental health social workers, the program offers assistance with service navigation, bridging support, counseling/therapy and diagnostic assessments.

Cold, Flu, and COVID-19 Clinic

  • The Cold, Flu and COVID-19 Clinic at the Thorncliffe Park Community Hub provides care for children aged three months to 17 years in East Toronto who are experiencing cold, flu and COVID-19 symptoms when their primary care provider is not available

  • This service will be expanding to adults aged 18 and older in East Toronto starting January 11, 2023

Community and Specialist Resources

  • Health Access Taylor Massey (HATM) is a healthcare, community and social services centre located in Crescent Town. HATM offers a range of services including COVID-19 testing, vaccine and prescriptions, mental health counseling and social services; click here for a complete list

  • Ocean eReferral continues to expand in East Toronto – if you are interested in using Ocean eReferral, please contact the eServices Team:eReferral@ehealthce.ca.

  • Become an EasT-FPN member to receive clinical and practice support, and opportunities to engage with the East Toronto community. Please fill out the Membership application form to become a member.

    • As part of the membership onboarding, members are encouraged to complete the following EasT-FPN Membership Intake Form, to provide us with key practice information, which will inform the EasT-FPN program and service planning.
