QIP Funding Opportunity to Reduce Cancer Screening Backlog

  • The Toronto Central Regional Cancer Program, Cancer Screening Team is providing up to $2,000 funding and additional in-kind support for primary care practices testing and/or implementing improvement ideas focused on reducing cancer screen backlog due to COVID-19.

Long Haul COVID-19 Program 

  • Unity Health Toronto has launched a new COVID-19 Recovered Outpatient Rehabilitation Program at Providence Healthcare that supports medically-stable patients experiencing non-urgent post-COVID-19 symptoms through an inter-professional team that includes an occupational therapist, physiotherapist and physiatrist. 

  • Please use the referral form and fax the completed referrals to the Admissions Office at 416-285-3759 and call the Admissions Office at 416-285-3744 with any questions.

SCOPE 

  • Register for SCOPE today and get rapid access to nurse navigation, general internal medicine, diagnostic imaging and more! SCOPE also provides access to orthopaedic services at MGH - click here for the fracture clinic weekly schedule and sub-specialist details. 

  • Please email SCOPE@tehn.ca with any feedback/suggestions for new services you would like to see added.

Free O2 Sat Monitor 

  • Primary care professionals can request oxygen saturation monitors through the Ministry of Health’s COVID@Home initiative that belong to their practice and can be loaned out to patients as required. Please complete the Oxygen Saturation Monitor Eligibility and Intake Form to request a monitor.

Digital Solutions 

  • Download Hypercare for easy and secure communication among community primary care providers and hospital care providers. Visit the EasT-FPN website for more information on digital solutions and Hypercare.

Ocean eReferral

  • Ocean eReferral continues to expand in East Toronto – enabling enhanced communication between primary care providers and specialists/organizations and simplifying the referral process. If you are interested in using Ocean eReferral, please contact the eServices Team: eReferral@ehealthce.ca.

MGH Rounds 

  • Virtual Medical Grand Rounds and Family Practice Rounds are continuing monthly, offering interesting topics and a chance to connect with your colleagues! Please email to get on the list for Family Practice Rounds and email for the next Medical Grand Rounds.

  • The EasT-FPN encourages you, if you have not already, to become a member to receive clinical and practice support and opportunities to engage with the East Toronto community. Please fill out the Membership application form to become a member.

Youth and Adult Mental Health Primary Care Survey 

  • In partnership with MGH, and other community organizations, the ETHP is redesigning the youth mental health experience for youth (ages 12-29), providers, and caregivers. Please complete the following survey that will inform how EasT-FPN primary care practitioners are currently supporting mental health in East Toronto enabling us to understand the interests and required support to build an integrated health system to support youth and their mental health. 

Hospitalist Work Opportunities

COVID-19 Assessment Centre/ Vaccine Clinics 

  • As community transmission of COVID-19 has decreased, fewer assessment community pop-ups will be operating. Please see the updated list of temporary COVID-19 testing sites in East Toronto and their hours here

  • Through the EasT-FPN, 80 family physicians and 27 specialists colleagues signed up to vaccinate at the Scotiabank Arena Winning Shot event on June 27th, 2021. Click here for the instructions on how to sign up for shifts at the CAC/pop up sites and Vaccination Clinics.
