AGM - September 29, 2021 from 7:00-9:00pm

  • Join your fellow EasT-FPN members at the 2nd Annual General Meeting. Attendees will participate in a wellness session, network with primary care colleagues, elect new EasT-FPN Board members & share in the accomplishments of the past year.

  • This year, attendees will receive a special catering package delivered to your door! Please pre-register using the following form by September 13, 2021 to ensure you receive your package.

  • Closer to the date, a meeting link and instructions will be shared with all those who register.

Join the Digital Working Group!

  • The EasT-FPN is forming a Digital Working Group and is looking for a nominee from your practice community or organization that can represent your needs on primary care digital priorities and digital integration into our wider OHT community. Members of the working group will be funded for their time. Read more about it here and direct your nominee to connect with us at contact@eastfpn.org, with a subject line “Nominee for Digital Working Group”.

MGH Child/Youth Outpatient Programs Referral Change

  • Starting August 9th, 2021, all referrals for the Child and Youth Outpatient Mental Health Services at MGH will be centrally processed by Mental Health T.O. (MHTO). This partnership will streamline the referral and intake process for patients and families to offer improved access to a range of community-based mental health services in their area through MHTO’s navigation services in addition to psychiatric consultation.

ETHP Funding for Micro Initiatives

  • There is funding available from the ETHP for eligible East Toronto family practices or engaged partner organizations who require small-scale funding to allow digital transformation of their business. Click here for more information and how to apply.

Board of Directors - Call for Nominations

  • As a not-for-profit organization that is focused on empowering the East Toronto communities, the EasT-FPN is seeking 4 Board of Director positions that will contribute to effective governance and leadership for the organization in a volunteer capacity. Ocean eReferral 

  • Ocean eReferral continues to expand in East Toronto – enabling enhanced communication between primary care providers and specialists/organizations and simplifying the referral process. If you are interested in using Ocean eReferral, please contact the eServices Team:eReferral@ehealthce.ca.

Youth and Adult Mental Health Primary Care Survey - Looking for family practice  physicians/clinicians to take new patients

  • Please complete the following survey that will inform how EasT-FPN primary care practitioners are currently supporting mental health in East Toronto enabling us to understand the interests and required support to build an integrated health system to support youth and their mental health. For those of you who completed the first iteration of the survey, we did not have a place for you to put your name! Please let us know.
