AGM - September 29, 2021 from 7:00-9:00pm

  • Join fellow EasT-FPN members at the 2nd Annual General Meeting. Participate in a wellness session, network with primary care colleagues, elect new EasT-FPN Board members, share in the accomplishments of the past year and provide input on our next initiatives.

  • This year, attendees will receive a special catering package delivered to your door! Please pre-register using the following form by September 13, 2021 to ensure you receive your food package.

  • Closer to the date, a meeting link and instructions will be shared with all those who register.

Board of Directors - Call for Nominations

  • As a not-for-profit organization focused on empowering the East Toronto communities, the EasT-FPN is seeking 4 Board of Director positions that will contribute to effective governance and leadership for our organization in a volunteer capacity. COVID-19 Outreach Centres: Testing and Vaccination Access for Patients

  • Anyone can receive COVID-19 testing, vaccinations and other COVID-19 support all in one spot at the new COVID-19 Outreach Centres! Click here for more information and locations/hours.

  • Find out how you can sign up for shifts at the COVID-19 Outreach Centres and Vaccination Clinics.

  • Looking for 4 physician leaders to provide clinical and administrative leadership in the East Toronto Health Partners (ETHP) COVID-19 Outreach Centres, to drive vaccination and assessment strategies through a community-based approach. There will be a Physician Leader – anchored in each of the 4 East Toronto Primary and Community Care Quadrants, which serve the ETHP (OHT) across 4 distinct geographies. This is a paid position and is responsible for providing direction and support to clinicians and collaborate with community providers within their assigned quadrant, as they implement integrated models of care, with the support of a strong project management team. Ocean eReferral 

  • Ocean eReferral continues to expand in East Toronto – enabling enhanced communication between primary care providers and specialists/organizations and simplifying the referral process. If you are interested in using Ocean eReferral, please contact the eServices Team:eReferral@ehealthce.ca.
