Register Today! EasT-FPN AGM - September 20th from 7-9pm

  • Join your fellow EasT-FPN members at the 4th Annual General Meeting on September 20th from 7-9pm! Attendees will elect new Board members, network with primary care colleagues and participate in an engagement session (takes place second half of the night!

OCEAN eReferral Updates

  • Several new sites are now actively accepting referrals via Ocean, including:

    • Joint Department of Medical Imaging (JDMI; which includes Sinai Health, University Health Network, and Women’s College Hospital) is accepting eReferrals for general radiography, CT, MRI, ultrasound, PET scans, mammography, and interventional radiology - these are searchable by the test type, the specific hospital site, or “JDMI”

    • Centre for Addiction and Mental Health (CAMH) is accepting e-referrals for its comprehensive mental health and substance use programs

    • MGH continues to accept eReferrals for the majority of its services, including Diagnostic Imaging (x-ray, ultrasound, mammography, nuclear medicine, bone density scans, CT, and MRI), Outpatient Adult Mental Health Clinic, and Child & Youth Mental Health services

    • Please contact us at contact@eastfpn.org if you are experiencing any particular challenges with Ocean eReferrals, or have feedback on specific sites or specialists in East Toronto that you would like to see come onboard with eReferral!

Community and Specialists Resources

  • The Toronto ABI Network helps connect people with Acquired Brain Injury (ABI) to the services they need. We suggest you:

  • Thorncliffe Park Youth Wellness Hub’s in-person services are now available by walk in during drop-in hours: Monday to Friday, 11 a.m. to 7 p.m. Services are free and open to everyone aged 12 to 25. Youth do not need a health card, family doctor or medical referral. More information can be found at ethp.ca/ThorncliffeYWHO. Please share this flyer with your colleagues and patients!

  • Register for SCOPE today and gain access to various SCOPE pathways such as nurse navigator, general internal medicine, medical imaging and more!

  • Ocean eReferral continues to expand in East Toronto – if you are interested in using Ocean eReferral, please contact the eServices Team:eReferral@ehealthce.ca.
