COVID-19 Vaccination Updates
Michael Garron Hospital (MGH) has outlined various eligibility groups for the COVID-19 vaccine. Click here for locations and let your patients know!
We are ONE community vaccinating patients of all ages, all over East Toronto. See how primary care clinics, through the East Toronto Family Practice Network (EasT-FPN) have collaborated with ETHP to deliver vaccines to our patients below. If your clinic is interested in vaccinating patients as well, please click here. The Primary Care Vaccine Planning Group is meeting every Friday at 4pm as a community of practice to support each other and troubleshoot challenges.
Survey: Re-designing Youth Mental Health Services in East Toronto
In partnership with MGH, and other community organizations, the ETHP is redesigning the youth mental health experience for youth (ages 12-29), providers, and caregivers. Please complete the following survey that will inform how EasT-FPN primary care practitioners are currently supporting mental health in East Toronto enabling us to understand the interests and required support to build an integrated health system to support youth and their mental health.
COVID-19 Response Forums
Primary Care COVID-19 Open Forum – August 25, 2021 6:00-7:00pm
Due to the high COVID numbers, and time constraints, the Primary Care forum will reconvene in August, we will review the COVID numbers at that time and discuss current primary care challenges.Retirement home (RH) Support Discussion Forum – June 8, 2021 5:00-5:30pm
Long-term Care (LTC) Support Discussion Forum – August 26, 2021 4:30-5:00pm
The LTC Discussion forums will now be held quarterly, with the next forum held on August 26th. LTC and RH forums connect physicians to MGH specialists including Dr. Jarred Rosenberg, MGH’s Nurse Led Outreach Team, Ontario Health’s Home and Community Care leads, TPH and RHRA representatives. Please see the May 11th RH and Apr 29th LTC summary documents.
COVID-19 Assessment Centre
Please encourage your community to get tested when they have symptoms. The EasT-FPN, in partnership with MGH have created the MGH COVID-19 Assessment Centre (CAC) and 9 pop-up sites. See the list of temporary CAC pop up sites and their hours on the web link.
