  • Thank you to everyone who attended the EasT-FPN AGM on September 20th! We elected new board members, approved new by-laws and participated in an engagement session on the Use of AI!

Toronto Community Hep C Program (TCHCP)

  • The TCHCP is a community-based collaborative care model that provides HCV support, education and treatment to individuals living with or at risk of acquiring Hepatitis C infection.

Patient Experience Study Opportunity

  • The Health System Performance Network (HSPN) is looking for participants in their study on the Patient-Reported Experience and Patient-Reported Outcome Measurement. Participants will be offered $750 in compensation.

Primary Care Operational Direction from Ontario Health

Hypercare Access Update

  • For family doctors and nurse practitioners who are current users of Hypercare and are credentialed at MGH, please ensure your credentialing is up to date

  • We are anticipating that MGH will be changing how family doctors and nurse practitioners who are not credentialed will be accessing Hypercare - stay tuned for updates

SCOPE and Ocean e-Referral Updates

  • Are you a SCOPE Physician/Nurse Practitioner? If so, you can now refer to SCOPE via Ocean e-Referral. Please go to Ocean health map, type SCOPE – East Toronto in the search bar, click send referral, and you will then be presented with all the services/ pathways that SCOPE in East Toronto offers. If you haven't already registered for SCOPE, please register here

  • Women’s College Hospital is now accepting referrals at the Bay Centre for Birth ControlNephrology Clinic and Toronto Anorectal Program (TARP)

  • MGH continues to accept eReferrals for the majority of its services, including Diagnostic Imaging, the Multidisciplinary Care Kidney Clinic, and General and Colorectal Surgery. 

  • Please contact us at contact@eastfpn.org if you are experiencing any particular challenges with Ocean eReferral, or have feedback on specific sites or specialists in East Toronto that you would like to see come onboard with eReferral!

  • Looking for a specialist to refer your patient? Please visit the Specialist and Community Resources page on the EasT-FPN website to find MGH and community specialists for multiple specialties. Please click on the specialty of your choice to jump to that specific section, or scroll down.

  • The Toronto Public Health website provides information on where to access free Rapid Antigen tests, as well as other information on testing.

  • A great resources for providers, patients, and caregivers to find health, social, and community services in Toronto. The detailed service listing includes languages, eligibility, accessibility and contact information.
