Immediate Action Required: Toronto Region Primary Care PPE Survey Update

If you are a group leader for an OHT, FHG, FHO, FHT, midwifery practice, or NPLC please forward this email to all your members as soon as possible. 

It is imperative that all practices in the Toronto Region Community Family Physicians and Nurse Practitioners complete this survey. One person can complete the survey on behalf of the practice.

Dear Family Physician or Nurse Practitioner:
 
Ontario Health (Toronto Region) is requesting primary care practices (including family medicine and NP clinics) to report on office availability and PPE needs. We recognize that there are significant demands on your time during this extraordinary situation.

This survey will:

  • take approximately 5-10 minutes to complete

  • provide us with the concise and local information we need to support primary care planning.

  • enable us to find short-term solutions to PPE shortages including how to allocate PPE resources when the region receives an allocation dedicated to primary care.


We request that each survey is completed on behalf of ONLY ONE practice location/site.

The survey will only accept one survey per IP address. Please forward this email to other practice sites for completion.

Survey results will only be shared with health system partners to enable the allocation and distribution of PPE to your practice.
 
Please tell us if you have a critical PPE shortage.  The Toronto  Region Supply Chain Team responds to critical requests when health service providers have 7 days or less PPE supply. Providers need to have exhausted all available supply chain options before making a request. The Supply Chain Team uses the Provincial Ethical Framework for prioritization of requests and can support by linking providers to PPE donations and other supply chain options.
